Expecting patients to adhere to their care plan is challenging when they lack trust in the system, have no support network and lack the knowledge to be advocates for their health.

With Truity™, multi-disciplinary teams provide patients with health equity consultations to increase adherence to their care plan. These teams include the right clinicians, interpreters, and family members to provide the support needed.

Here’s how it works:

After meeting with their provider, patients receive a prompt to log in to their patient portal.

In the portal, they can download a personalized insight report with health literacy appropriate plain language articles and recommendations related to their specific conditions.

Your patients will receive nudges and reminders to schedule a health equity consultation.

If the health consultation occurs, the care team discusses the patient’s conditions, provides follow up visits and tests, and educational resources to keep them engaged in their care plan.
