Use patient-specific coaching in your health equity initiative

Expecting patients to adhere to their care plan is challenging when they lack trust in the system, have no support network and lack the knowledge to be advocates for their health.

Health systems can utilize culturally competent coaches who will meet with patients and provide a detailed review of their insights report and deliver guidance on how a they can be more engaged to advocate for their care.

It’s time to use patient-specific coaching in your health equity initiative to break the chain of systemic inequity, increase community trust, and improve outcomes for diverse patient populations.
